Welcome to the Bovey Tracey & Chudleigh Practice. We shall be pleased to register you with the practice if you reside within our practice area, and complete our Application to Register an NHS Patient Form. In line with current NHS guidance we shall also require Photo I.D. (e.g. Passport or Driving Licence) and proof of address (e.g. utility bill).
Once registered, we invite all new patients to attend for a New Patient Medical with a member of our Practice Nurse Team. This is a good opportunity for a ‘MOT’ and will also ensure your continuing care if you suffer from any chronic disease such as asthma, diabetes or heart disease.
Currently we are registering all new patients with Dr Liam Kinsella, Dr Deborah Hughes or Dr Paul Russell at Riverside Surgery, and Dr Emily Eracleous or Dr Daniel Thomas at Tower House Surgery.
If you are currently taking repeat medication we ask that you let us have a copy of your repeat medication list from your previous surgery so that we can arrange to have this added to your records. Once reviewed and approved by your doctor, repeat prescriptions may be obtained by sending your request slip to the surgery or putting in a request via SystmOnline – please allow two working days between requesting and collecting. We regret we are unable to accept prescription requests over the telephone.
Please also find enclosed our Practice Booklet, information explaining our appointment system and Patient Newsletter which contain some useful information and contact numbers, including arrangements for out-of-hours.
